Renewing your Class 4 Gambling Licence
If you are a pub, club or incorporated society that operates gaming machines (aka "pokies"), the Gambling Act 2003 requires you to renew your Class 4 Gambling licence on an annual basis through the Department of Internal Affairs (DIA).
The Gaming Machine Account Summary form is a mandatory document through which you can submit all of the relevant financial information that the DIA requires. This information will demonstrate how your gaming machine proceeds have been managed, including expenses and distributions to authorised community purposes, as well as a forecast for the next financial year ahead of you. Depending on the nature of your organisation, your gaming machine account summary will take one of the following forms:

This is a form used by multi-venue clubs and other societies to renew their Class 4 Gambling Licences.

This form is used by single-venue clubs to renew their Class 4 Gambling Licences.
The DIA will usually require that an Independent Chartered Accountant holding a Certificate of Public Practice verifies the information provided in your renewal application.
